Please use the old Long Term Care entrance on the corner of Spruce St. and Scott Ave.

Parent/guardian must be present to provide consent. Masks are required; everyone will be screened upon entrance. Athletes should wear loose- tting clothing (shorts, t-shirts) for physicals.
This clinic is for sports physicals only. All other health concerns will require another appointment.

FREE sports physicals are o ered on a rst-come, rst- served basis. All other dates may be scheduled for a fee of $30. (509) 447-3139
